New patient paperwork is completed through our secure patient portal. If you prefer paper forms, our front desk will email or mail them ahead of your visit.

What to bring

• Photo ID and insurance card
• Any prior imaging (X-ray, MRI, CT) on a CD or via the portal
• A list of current medications
• Notes on what makes your symptoms better or worse
