Bio Cuff Repair is a biologically augmented rotator cuff repair Dr. Kovack uses for larger tears, thinner or fatty-infiltrated tendon tissue, and revision cases where a standard repair alone carries a high re-tear risk.

The procedure is performed arthroscopically through small incisions. A dermal allograft (CuffMend) is placed over the repair to immediately strengthen it and to scaffold new, healthier tendon tissue as it heals.

Why biologic augmentation matters

Traditional rotator cuff repair stitches the torn tendon back to the bone — but the tendon itself can stay weak, which is why re-tears are common in larger or older tears. CuffMend dermal allograft can boost initial repair strength by more than 60% and meaningfully lower the chance of re-tear.

Over time the graft incorporates into the repair, helping new, stronger tendon tissue form and last.

Who it's for

Patients with large or massive tears, thinner tendon tissue on MRI, a prior failed repair, or higher physical demand are typical candidates. Dr. Kovack confirms whether biologic augmentation is appropriate intraoperatively based on tendon quality.

Recovery & What to Expect

Recovery mirrors a standard rotator cuff repair: roughly 4–6 weeks in a sling followed by 4–6 months of structured physical therapy. Biologic augmentation is intended to improve durability of the repair, not shorten the protocol.

A standard repair reattaches the tendon to the bone with sutures. Bio Cuff Repair adds a dermal allograft over that repair to immediately strengthen it and promote healthier, more durable tendon healing.
Is the procedure outpatient?
Yes. Bio Cuff Repair is performed arthroscopically on an outpatient basis — you go home the same day.
Will my recovery be longer than a standard repair?
No. The post-op protocol is essentially the same as a standard rotator cuff repair. The graft is intended to make the repair more durable, not to lengthen recovery.
